Bring your insurace card and a photo ID.
Be sure to check with your insurance provider:
- If you have Chiropractic coverage
- How many visits your insurance will cover per calendar year
- What your co-pay is
- If you have a dectuctable with your plan and how much needs to be met
If capable, download and complete the appropriate Chiropractic Care paperwork
If your insurance does not provide Chiropractic coverage, don't worry, we offer very affordable self pay rates.
Chiropractic Care self pay rate:
- Initial evaluation and treatment (40-60 minutes) $90
- Follow up visits (15-20 minutes) $50
Physical Therapy
Bring your insurace card, photo ID, and the physical therapy prescription from your doctor
Be sure to check with your insurance provider:
- If you have Physical Therapy coverage
- How many visits your insurance will cover per calendar year
- What your co-pay is
- If you have a dectuctable with your plan and how much needs to be met
If capable, download and complete appropriate Physical Therapy paperwork
If you exhaust your Physical Therapy benefits, don't worry, we offer very affordable self pay rates.
Physical Therapy self pay rate:
- Initial evaluation and treatment (60 minutes) $90
- Follow up visits (30 minutes) $50

Auto and Work Related Injuries
If you were involved in a car accident, please bring with you a copy of the accident report, claim number, and your insurance policy declarations page.
Patient's involved in a work related injury must supply the claim number and insurance company information in order to initiate an initial evaluation. If your work related injury occurred in Massachusetts, valuable information may be obtained at www.mass.gov/dia
